Key Responsibilities
- Conduct health risk assessments and gather information from members and stakeholders
- Coordinate care plan goals and interventions while maintaining documentation
- Assist in providing health education and improve member experiences with the organization
Required Qualifications
- High School Diploma or GED is required
- Minimum of two years of experience in community settings with at-risk populations is preferred
- Proficient in Microsoft Office applications
- Experience working with diverse cultures and communities
- Ability to work independently and collaboratively in a team environment
