Job Summary

A company is looking for a Community Health Worker.

Key Responsibilities

• Conduct access to care assessments over the phone with KY Medicaid members and input responses into the Community Care database

Required Qualifications

• Experience as a Community Health Worker, Healthcare Support Representative, or Medicaid Support Representative
• Experience or training in motivational interviewing techniques
• Proven ability to work effectively with diverse cultural, ethnic, and socioeconomic backgrounds
• Comfortable using digital tools and platforms, including case management software and EMRs
• KY Medicaid experience and familiarity with SDOH needs preferred
