Massachusetts Licensed Community Health Worker

Job Summary

A company is looking for a Community Health Worker to provide care coordination to individuals with psychiatric conditions and/or substance use disorders.

Key Responsibilities

• Provide outreach and engagement to individuals referred to the Care Coordination program
• Coordinate and contribute to the completion of required assessments to identify member needs
• Design personal crisis management plans in collaboration with team clinicians and members

Required Qualifications

• Minimum of 3 years care management experience preferred, especially with Serious Mental Illness (SMI) and/or Substance Use Disorder (SUD)
• BA/BS in a human-services related field preferred; experience may substitute for academic training
• Preference for bi-lingual/bi-cultural applicants and those with lived experience of psychiatric conditions or substance use disorders
• Certified Community Health Worker (CHW) preferred but not required
• Must have access to a reliable personal vehicle and be at least 21 years of age
