Job Summary

A company is looking for a Telephonic Nurse Case Manager - Remote.

Key Responsibilities

• Make outbound calls to assess members' current health status
• Provide patient education and coordinate care for members
• Document and track findings while identifying gaps in treatment plans

Required Qualifications

• Current, unrestricted RN license in New Jersey
• 2+ years of RN experience in a hospital or acute care setting
• 1+ years of experience working with the elderly or physically disabled
• 1+ years of experience in long-term care, home health, or public health
• Ability to navigate a Windows environment and utilize Microsoft Office applications
