A company is looking for a Care Manager, LPN.
Key Responsibilities
- Deliver telephonic care management for patients enrolled in Value-Based Care Programs
- Support quality gap closure and coordinate care with providers and the interdisciplinary team
- Communicate patient health updates and treatment plans to caregivers and providers
Required Qualifications, Training, and Education
- Active, unrestricted compact LPN license
- 2+ years of nursing experience in various care settings
- Knowledge of pathophysiology and treatment protocols for common health diagnoses
- Proficient computer skills and ability to adapt to technology platforms
- Ability to work independently in a remote environment
