New York Licensed Care Manager

Job Summary

A company is looking for a Care Manager (RN) to develop and facilitate care management activities for members with physical health needs.

Key Responsibilities

• Evaluate member needs and recommend care plans to address barriers to accessing appropriate care
• Coordinate with providers and community resources to ensure members receive adequate person-centered care
• Monitor member progress and update care plans as necessary to meet evolving needs

Required Qualifications

• Degree from an Accredited School of Nursing or a Bachelor's degree in Nursing
• 2 - 4 years of related experience in care management
• New York State Registered Nurse license required
• Ability to collaborate with various healthcare providers and community resources
• Experience in documenting and maintaining member information for compliance purposes
