New York RN Care Coach

Job Summary

A company is looking for a New York Registered Nurse Care Coach.

Key Responsibilities:

• Utilize care management software to conduct monthly calls with Medicare patients with chronic conditions
• Build rapport with patients and coach them on self-management strategies and health goals
• Implement and improve Plans of Care, including updating medications and connecting patients with community resources

Required Qualifications:

• Current, unrestricted New York RN license
• 3 or more years' experience as a Registered Nurse
• Proficiency with EHRs and web-based applications
• Self-directed with the ability to work independently
• Passion for nursing and improving patient outcomes
