Job Summary

A company is looking for a Care Manager.

Key Responsibilities

• Develops and monitors Tailored Care Management care plans and Individual Support Plans (ISPs) for assigned caseloads
• Coordinates care team meetings and engages with members and families to ensure service provision and follow-through
• Promotes comprehensive care by connecting members with various support services and addressing unmet health-related needs

Required Qualifications, Training, and Education

• Two years of experience as a Care Manager, Case Manager, or Care Coordinator preferred
• Must meet all agency requirements for pre-employment and those required by the state of NC
• A license or certification in a relevant human service profession, or a degree in a human service field with specified experience
• Experience with the IDD population is required, with specific conditions for LTSS needs
• Ability to perform work with a high degree of quality and autonomy
