Job Summary

A company is looking for a Nurse Practitioner - PRN Telehealth.

Key Responsibilities

• Conduct Adult/Geriatric Assessments, including medical history, diagnosis, treatment, and documentation
• Collaborate with healthcare professionals and caregivers to ensure comprehensive patient care
• Provide services through Home Visits, Skilled Facility Visits, and Telehealth Visits

Required Qualifications

• Master's Degree or equivalent experience and NP licensure
• Current RN and NP licensure with prescription authority in the state of practice
• Board certification in a Matrix approved specialty
• Current BLS, ACLS, or CPR certification
• Minimum of 1 year experience as a Nurse Practitioner
