Job Summary

A company is looking for a Remote LPN Admissions Coordinator Long-Term Care & Rehabilitation.

Key Responsibilities

• Support admissions and transitions of care for members entering long-term care or rehabilitation facilities
• Collect and review clinical documentation and eligibility information for RN or physician review
• Serve as a liaison between health plans, facility admissions teams, and providers to ensure timely placements

Required Qualifications

• Active, unrestricted LPN license in the State of Ohio
• At least 2-3 years of nursing experience in long-term care, rehabilitation, or admissions/intake coordination
• Familiarity with Ohio Medicaid processes and facility admissions workflows
• Proficiency with EMR systems and care coordination tools
• Ability to work independently and remotely in a fast-paced environment
