Job Summary
A company is looking for an LTSS Coordinator II to monitor and promote effective utilization of long-term services and supports through clinical review and benefits management.
Key Responsibilities
- Act as a point of contact within the interdisciplinary care team to support coordination of long-term services and supports
- Complete reviews for NF level of care determinations, respite care, waiver services, and home modifications
- Coordinate care with internal and external stakeholders to facilitate timely and cost-effective discharge planning
Required Qualifications, Training, and Education
- Completion of an accredited Registered Nurse (RN) degree program or degree required for Social Worker licensure in Ohio
- Bachelor's Degree in Nursing (BSN) or equivalent healthcare degree is preferred
- Minimum of three (3) years of experience in case management and/or managed care is required
- Experience in a Medicare and/or Medicaid managed care environment is preferred
- Current, unrestricted Registered Nurse (RN) or Licensed Social Worker (LSW) licensure in Ohio is required
