Job Summary

A company is looking for a Pediatric Case Manager - Registered Nurse for a remote telehealth position.

Key Responsibilities

• Assess, plan, implement, and coordinate case management activities to evaluate and enhance members' medical needs and overall wellness
• Conduct evaluations using clinical tools and collaborate with interdisciplinary teams to address complex health and social indicators
• Utilize motivational interviewing skills to engage members and discern their health status and needs

Required Qualifications

• Registered Nurse with an unrestricted nursing license and multi-state/compact privileges
• 3+ years of clinical/nursing practice experience
• 3+ years of pediatric experience
• 1+ year of experience in care coordination or discharge planning
• Associate's degree in nursing required; BSN preferred
