New Jersey Licensed RN Case Manager

Job Summary

A company is looking for an RN Case Manager for the Care Connect Program focusing on High Risk Maternity/NICU.

Responsibilities:

• Assess patients' clinical needs to ensure appropriate level of care and length of stay
• Coordinate and implement care plans while monitoring services for effectiveness and appropriateness
• Document care activities accurately and mentor/train new RN staff as needed

Qualifications:

• High School Diploma/GED required; Bachelor's degree preferred or relevant experience in lieu of degree
• Minimum of two (2) years clinical experience required
• At least two (2) years' experience with healthcare payer experience required
• Active Unrestricted NJ RN License or active Compact License required
• CCM certification preferred for Clinical Advocate roles
