Key Responsibilities
- Engage with members through various communication methods to assess their needs and provide support
- Connect members with healthcare providers, resources, and schedule health and wellness visits
- Conduct survey assessments to evaluate changes in health and assist with advanced care planning
Required Qualifications, Training, and Education
- Experience in a remote role with a focus on member or customer support
- Comfortable using technology, including video conferencing and electronic health records
- Ability to work in a quiet and secure environment to maintain member privacy
- Background in healthcare or related fields is preferred
- Willingness to undergo virtual onboarding and training
