Job Summary

A company is looking for a Care Partner (8:30AM - 5:00PM EST Shift).

Key Responsibilities

• Engage with members through various communication methods to assess their needs and provide support
• Connect members with healthcare providers, resources, and schedule health and wellness visits
• Conduct survey assessments to evaluate changes in health and assist with advanced care planning

Required Qualifications, Training, and Education

• Experience in a remote role with a focus on member or customer support
• Comfortable using technology, including video conferencing and electronic health records
• Ability to work in a quiet and secure environment to maintain member privacy
• Background in healthcare or related fields is preferred
• Willingness to undergo virtual onboarding and training
