Job Summary

A company is looking for a Registered Nurse Home Visitor - Community Based.

Key Responsibilities

• Coordinate care and conduct home visits for families enrolled in the Nurse Family Partnership
• Assess the physical, emotional, social, and environmental needs of women and their families
• Provide education, guidance, and support regarding pregnancy, infants, and toddlers

Required Qualifications

• Graduate from a Specialty Training Program (Required)
• Bachelor's Degree in Nursing (Preferred)
• Minimum of 2 years of nursing experience (Preferred)
• Licensed Registered Nurse in Pennsylvania
• Basic Life Support Certification
