Job Summary
A company is looking for an Assistant Service Coordinator (Remote).
Key Responsibilities
- Assist Enrollees in navigating and accessing long-term services and supports (LTSS) and eliminate duplication of services
- Collect and track program data to ensure compliance with reporting and regulatory deadlines
- Coordinate with various stakeholders to ensure seamless delivery of physical, behavioral, and support services for Participants
Required Qualifications
- Associate's degree with at least one year of professional experience in the field, or a Bachelor's degree in social work, psychology, or related fields preferred
- Experience working with people with disabilities or seniors in need of LTSS preferred
- Cultural competency and sensitivity to diverse backgrounds
- Ability to work independently in a virtual setting and make outbound calls
- Knowledge of home and community-based service systems
