Job Summary

A company is looking for a Bilingual Care Coordinator - SNP.

Key Responsibilities

• Perform assessments and screenings to provide appropriate interventions and referrals
• Act as a triage contact for members and providers, addressing access issues for beneficiaries
• Coordinate care utilizing member benefits and community resources to enhance health outcomes

Required Qualifications

• High School Diploma required; some college preferred
• Bilingual in English and Spanish, Mandarin, Cantonese, Tagalog, Korean, or Vietnamese
• 1 or more years of experience in healthcare or customer service
• General knowledge of medical terminology and local community resources for seniors
• Understanding of CMS and DHCS regulations
