Key Responsibilities
- Provides outreach to members to support care plans and connect them with resources
- Serves as front-line support for member and provider inquiries and concerns
- Documents and maintains non-clinical member records in compliance with regulations
Required Qualifications
- High School diploma or GED
- 1 - 2 years of related experience
- Knowledge of health plan activities and community resources
- Ability to document and maintain records according to state regulations
- Understanding of Social Determinants of Health (SDOH) needs
