Job Summary
A company is looking for a Care Management Support Coordinator II to support administrative care management activities.
Key Responsibilities
- Perform outreach to members via phone to assist with care plan next steps and resources
- Connect members to health plan and community resources to ensure high-quality customer care
- Document and maintain non-clinical member records in compliance with state and regulatory requirements
Required Qualifications
- High School diploma or GED required
- 1 - 2 years of related experience preferred
- Experience in a call center or high volume of calls is required
- 2-3 years of healthcare medical office, facilities, and administration duties strongly preferred
- Knowledge of existing benefits and resources locally is a plus
