Job Summary

A company is looking for a Care Management Support Coordinator II to support administrative care management activities.

Key Responsibilities
  • Perform outreach to members via phone to assist with care plan next steps and resources
  • Connect members to health plan and community resources to ensure high-quality customer care
  • Document and maintain non-clinical member records in compliance with state and regulatory requirements
Required Qualifications
  • High School diploma or GED required
  • 1 - 2 years of related experience preferred
  • Experience in a call center or high volume of calls is required
  • 2-3 years of healthcare medical office, facilities, and administration duties strongly preferred
  • Knowledge of existing benefits and resources locally is a plus
