Job Summary

A company is looking for a Care Manager.

Key Responsibilities

• Engage members and their support systems through assessments and care planning
• Facilitate inter-disciplinary care team meetings and coordinate care among team members
• Monitor and evaluate member satisfaction and progress on individualized care plans

Required Qualifications, Training, and Education

• Nursing degree from an accredited program or Bachelor's degree in a healthcare field
• Minimum of three (3) years of experience in nursing, social work, or healthcare profession
• Preferred advanced degree associated with clinical licensure
• Preferred three (3) years of Medicaid and/or Medicare managed care experience
• Current unrestricted clinical license as a Registered Nurse, Social Worker, or Professional Clinical Counselor
