Job Summary

A company is looking for a Social Care Navigator - Mandarin/Cantonese Speaking.

Key Responsibilities

• Use motivational interviewing to empower members in achieving health and personal goals
• Conduct outreach to members to assess their needs and provide tailored support
• Identify and refer members to local resources and collaborate with internal teams for continuity of care

Required Qualifications

• 2+ years of experience in Care Navigation, Social Work, Health Coaching, or related support roles
• Fluency in Mandarin or Cantonese is required
• Experience with systems like Medicaid, SNAP, or public housing
• Experience working with low-income individuals or those with chronic conditions
• Comfortable using digital tools for organization and documentation
