Job Summary

A company is looking for a Senior Director, Clinical Excellence (Post Acute, VBC).

Key Responsibilities

• Oversee the delivery of the Post Acute Care Program ensuring seamless patient transitions and improved outcomes
• Drive optimal performance of the PAC clinical team through engagement and development
• Collaborate with stakeholders to ensure high-quality PAC care delivery and compliance with regulations

Required Qualifications

• Unrestricted RN, NP, or PA state license
• 10+ years of clinical leadership experience
• Master's degree or higher in a related field preferred
• Experience in value-based care models and establishing KPIs
• Proven track record in improving clinical quality and outcomes
