Key Responsibilities

• Provide patient education and support for medication adherence and disease management through telephonic outreach
• Develop, implement, and evaluate comprehensive care plans for patients with multiple disease states
• Collaborate with healthcare professionals to enhance patient care and experience

Required Qualifications

• Current unrestricted Registered Nurse license in Texas or Arizona, with multi-state licensing capability
• Minimum of 3 years of clinical RN experience
• Proficiency in Microsoft Office applications, including Word, Excel, and Outlook
• RN Diploma, Associate's, or Bachelor's degree in Nursing
• Candidate must reside in Texas or Arizona
