Transitions Care Coordinator

Job Summary

A company is looking for a Transitions Care Coordinator.

Key Responsibilities

• Assist care coordination for patients discharged from ER visits and acute care facilities
• Act as a liaison between care teams and external medical providers
• Monitor admissions and clinical outcomes, providing feedback as necessary

Required Qualifications

• At least 1 year of patient-facing experience in a healthcare setting
• Preferred experience as a hospital case management assistant or home health care coordinator
• Experience working with senior patients preferred
• Understanding of Mac iOS and Google Suite
• Spanish speaking preferred
