Arizona Licensed Population Health Assistant

Job Summary

A company is looking for a Population Health Assistant to provide clinical support and improve healthcare quality.

Key Responsibilities

• Coordinate services for patients and ensure appropriate referrals and appointments
• Monitor health plan members and assist with patient navigation through the healthcare system
• Document patient interactions and interventions accurately in the medical record

Required Qualifications

• High school diploma/GED or equivalent working knowledge
• Three years of experience in healthcare as a Nursing Assistant, Medical Assistant, or similar role
• Current CNA, CHW, MA certification, or LPN license may be required
• Understanding of medical terminology and clinical processes
• Strong organizational and time management skills
