Job Summary

A company is looking for a Bilingual Care Partner (11:30AM - 8:00PM EST Shift).

Key Responsibilities

• Engage with members through various communication methods to assess their needs and provide support
• Connect members with healthcare providers, resources, and schedule health and wellness visits
• Conduct survey assessments to evaluate changes in members' health and assist with advanced care planning

Required Qualifications and Education

• At least 2 years of experience in a patient-facing or member-facing role
• Experience in a metrics-driven, feedback-oriented environment
• Baseline knowledge of healthcare, preferably with experience in a healthcare setting
• Bilingual in Spanish
• Comfortable with technology and remote work environments
