Job Summary
A company is looking for a Care Connector to provide program interventions under the guidance of a supervisor.
Key Responsibilities
- Support and communicate with Members to identify needs and provide information on healthcare access and preventive health interventions
- Assist Members with non-clinical functions, including linking to community resources and scheduling appointments
- Maintain accurate documentation of Member interactions in the electronic care management platform
Required Qualifications
- High School Diploma or equivalent required
- Minimum of two years of work experience in a healthcare environment required
- At least one year of telephonic customer service experience, preferably in healthcare
- Managed care experience preferred
- Strongly prefer training or certification as a Medical Assistant, Home Health Aide, Nursing Assistant, or similar healthcare para-professional
