Job Summary
A company is looking for a Care Coordinator who will support members through telephonic communication and collaboration with clinical teams.
Key Responsibilities:
- Handle inbound and outbound calls to support members
- Complete intake screening questionnaires and initial program assessments with members
- Conduct cold calls to engage members and assist with scheduling appointments
Required Qualifications:
- 2+ years of healthcare/clinical experience
- Call agent experience is preferred
- Basic understanding of EHR/EMR applications
- Proficiency in Microsoft Office
- Must live in the United States
