New York Licensed Care Manager

Job Summary

A company is looking for a Care Manager fluent in English and Cantonese for a remote position.

Key Responsibilities:

• Advocate and educate members on services and self-management techniques
• Conduct assessments and develop care plans in collaboration with healthcare providers
• Coordinate care among service providers and document all member interactions

Required Qualifications:

• NYS RN or LCSW/LMSW (any state) for Medical Care Management
• 3 years of work experience in a mental/behavioral health or addictions setting for Behavioral Health Care Management
• 1 year of pediatric clinical field experience for PEDS positions
• Fluency in Cantonese
• Experience managing member information in a shared network environment
