Colorado Licensed Nurse Care Manager

Job Summary

A company is looking for a Nurse Care Manager who will work collaboratively with healthcare teams to promote positive patient outcomes.

Key Responsibilities

• Assess, plan, implement, coordinate, monitor, and evaluate care options and services for patients
• Establish collaborative partnerships with patients to support their health care decisions and resource utilization
• Facilitate communication among healthcare team members and involve patients in the decision-making process

Required Qualifications

• Graduate of an accredited Registered/Professional Nursing program
• State licensure as a Registered Nurse (RN)
• Bachelor's degree in Nursing (preferred)
• Certified Case Manager (CCM) (preferred)
• 2 years' nursing experience in outpatient care and 1 year in Care Management (preferred)
