Job Summary

A company is looking for a Pediatric and Adult Case Manager - Registered Nurse - Remote - Telehealth.

Key Responsibilities

• Assess, plan, implement, and coordinate case management activities for members to evaluate their medical needs
• Use clinical tools to conduct evaluations and apply clinical judgment to address health and social barriers
• Collaborate with supervisors and stakeholders to overcome barriers and ensure compliance with regulatory policies

Required Qualifications

• Registered Nurse with an unrestricted nursing license and multi-state/compact privileges
• 3+ years of clinical/nursing practice experience
• 3+ years of pediatric experience
• 1+ year of experience in care coordination or discharge planning
• High-speed internet access and proficiency with MS Office Suite applications
