Compact RN Care Manager

Job Summary

A company is looking for a Care Manager (RN) to develop and facilitate complex care management activities for members.

Key Responsibilities

• Evaluate member needs and develop personalized care plans
• Coordinate care among providers and address social determinants of health
• Provide ongoing follow-up and resource support to members and caregivers

Required Qualifications

• Degree from an Accredited School of Nursing or Bachelor's degree in Nursing
• 2 - 4 years of related experience
• RN - Registered Nurse State Licensure and/or Compact State Licensure required
• Comfortable handling high call volumes and adapting to workflow changes
• Willingness to cover assignments or provide emergency coverage as needed
