Part Time LPN Care Coordinator

Job Summary

A company is looking for a Part Time LPN/LVN Care Coordinator, fully remote, to manage patient interactions and administrative tasks in a palliative care program.

Key Responsibilities

• Educate patients about palliative care and schedule appointments
• Manage clinical files, document patient encounters, and triage needs
• Follow up with patients to ensure quality care and close completed cases

Required Qualifications

• Minimum 3+ years as an LPN
• Minimum 3+ years of experience reading and updating clinical notes
• Minimum 1+ years of relevant experience in a customer service call center
• Comfortable working with technology in a dynamic environment
• Spanish-speaking is a plus but not required
