Job Summary
A company is looking for a Care Management Support Coordinator II.
Key Responsibilities
- Conducts telephonic outreach to members for enrollment in case management services and scheduling preventative care services
- Acts as a point of contact for members and providers to address inquiries and concerns related to care plans and resources
- Documents and maintains non-clinical member records in compliance with state and regulatory guidelines
Required Qualifications
- High School diploma or GED
- 1 - 2 years of related experience
- Knowledge of health plan activities and resources
- Understanding of Social Determinants of Health (SDOH) needs
- Ability to comply with policies and standards
