Job Summary

A company is looking for a Care Management Support Coordinator II.

Key Responsibilities
  • Conducts telephonic outreach to members for enrollment in case management services and scheduling preventative care services
  • Acts as a point of contact for members and providers to address inquiries and concerns related to care plans and resources
  • Documents and maintains non-clinical member records in compliance with state and regulatory guidelines
Required Qualifications
  • High School diploma or GED
  • 1 - 2 years of related experience
  • Knowledge of health plan activities and resources
  • Understanding of Social Determinants of Health (SDOH) needs
  • Ability to comply with policies and standards
