Engagement Specialist

Job Summary

A company is looking for an Engagement Specialist (REMOTE).

Key Responsibilities

• Serve as a primary contact for patients in the Palliative Care at Home program
• Schedule initial visits and collect patient information to determine eligibility
• Educate patients and providers about the benefits of Palliative Care at Home

Required Qualifications

• High school diploma or GED
• Two (2+) years of experience in a healthcare setting, preferably in palliative care, chronic care, or hospice
• Familiarity with Electronic Medical Records

Preferred Qualifications

• Bachelor's degree or three (3+) years of relevant professional experience
• Experience in healthcare community outreach or telephonic sales
• Familiarity with medical terminology
• Bilingual (English and either Spanish or Chinese)
