A company is looking for an Engagement Specialist (REMOTE).
Key Responsibilities
- Serve as a primary contact for patients in the Palliative Care at Home program
- Schedule initial visits and collect patient information to determine eligibility
- Educate patients and providers about the benefits of Palliative Care at Home
Required Qualifications
- High school diploma or GED
- Two (2+) years of experience in a healthcare setting, preferably in palliative care, chronic care, or hospice
- Familiarity with Electronic Medical Records
Preferred Qualifications
- Bachelor's degree or three (3+) years of relevant professional experience
- Experience in healthcare community outreach or telephonic sales
- Familiarity with medical terminology
- Bilingual (English and either Spanish or Chinese)
