Job Summary
A company is looking for a Care Management Support Coordinator.
Key Responsibilities
- Perform outreach to members and provide support for care plan next steps and resource connections
- Serve as the front-line support for member and provider inquiries, explaining care plan procedures and protocols
- Document and maintain non-clinical member records in compliance with state and regulatory requirements
Required Qualifications
- High School diploma or GED required
- 1 - 2 years of related experience preferred
- Knowledge of health plan activities and community resources
- Experience with Social Determinants of Health (SDOH) needs
- For specific plans, telephonic interaction with members is required
