• Provides behavioral health, medical, and psychosocial assessments to develop appropriate care plans
• Facilitates communication and coordination among members, families, and healthcare professionals

Required Qualifications

• Master's Degree in Social Work or related field
• Minimum of 3 years of work experience in a relevant setting
• Clinical Social Worker License (LCSW) required
• Licensed Mental Health Counselor (LMHC) preferred
