Job Summary

A company is looking for a Care Manager (RN) to support their Ambetter Marketplace members remotely.

Key Responsibilities

• Evaluate member needs and barriers to care, recommending plans for optimal outcomes
• Develop and collaborate on ongoing care plans, coordinating with providers and community resources
• Monitor member status and progress towards care plan goals, providing education and support as needed

Required Qualifications

• Degree from an Accredited School of Nursing or a Bachelor's degree in Nursing
• 2 - 4 years of related experience in case management, home health, or managed care
• Bedside nursing experience in med surg/acute care
• Georgia compact nursing license required
• Registered Nurse (RN) state licensure or compact state licensure required
