Job Summary

A company is looking for a Health Navigator to join their SeniorCare team.

Key Responsibilities
  • Conduct educational calls to advise members on benefits and services, completing health needs assessments
  • Encourage members to comply with care plans and assist in scheduling appointments
  • Manage system work queues and screen members for eligibility and case activities


Required Qualifications
  • Associates Degree in education, communication, or health-related field or equivalent work experience
  • 2 years of experience in a customer service support role
  • Ability to work independently and in a team environment
  • Knowledge of medical terminology
  • Experience managing multiple tasks and setting priorities
