Case Management Analyst

Job Summary

A company is looking for an Analyst, Case Management - Work From Home.

Key Responsibilities

• Assess members' needs and provide information and referrals to appropriate resources
• Document interactions accurately in electronic records to meet compliance and regulatory standards
• Support members during enrollment calls and assist in problem-solving when services are unavailable

Required Qualifications

• At least 2 years of experience in healthcare, nonprofit, or community-based services
• Experience handling high call volumes and working with auto-dialer systems
• Preferred experience in information & referral, health coaching, or care management
• Bachelor's degree or equivalent work experience
• Bilingual (Spanish) is preferred
