Job Summary
A company is looking for a Health Navigator, Member Outreach.
Key Responsibilities
- Conduct educational telephone calls to advise members on available benefits and services, completing health needs assessments and referring to appropriate programs
- Reach out to members with gaps in care, encouraging compliance and assisting with provider location and appointment scheduling
- Manage system work queues, screening members for eligibility and assigning them to clinical teams for outreach and intervention
Required Qualifications
- Associates Degree in education, communication, or health-related field or equivalent work experience
- 2 years of experience in a customer service support role is required
- Ability to work independently with minimal supervision
- Knowledge in medical terminology
- Proficient interpersonal and organizational skills
