Job Summary

A company is looking for a Health Navigator, Senior Care Outreach.

Key Responsibilities
  • Conduct educational calls to advise members on benefits and services, completing health needs assessments
  • Encourage members to address gaps in care and assist with provider location and appointment scheduling
  • Manage system work queues and facilitate research on inquiries related to member programs
Required Qualifications
  • Associates Degree in education, communication, or health-related field, or equivalent work experience
  • 2 years of experience in a customer service support role
  • Ability to work independently and in a team environment
  • Knowledge of medical terminology
  • Digital literacy assessment required
