Job Summary

A company is looking for a Community Health Worker with Kentucky Medicaid experience.

Key Responsibilities

• Conduct access to care assessments over the phone with KY Medicaid members
• Enter assessment responses into the Community Care database
• Support clients in navigating healthcare resources and services

Required Qualifications

• Experience as a Community Health Worker, Healthcare Support Representative, or Medicaid Support Representative
• KY Medicaid experience preferred
• Experience with Social Determinants of Health (SDOH) needs preferred
• Proficiency in using digital tools and platforms such as case management software and EMRs
• Organizational skills to manage multiple priorities effectively
