Licensed Social Programs Manager

Job Summary

A company is looking for a Social Programs Manager.

Key Responsibilities

• Provide direct clinical care and resource management for patient panels in High Risk programs
• Develop, implement, and evaluate evidence-based practices for chronic illness and case management
• Supervise and mentor social worker and social program team members

Required Qualifications

• At least 5 years of clinical experience, with 3 or more as a leader in High Risk Care or similar programs
• At least 2 years of experience managing teams
• LCSW or LMSW with an active license in CO, AZ, GA, WA, or TX, and willingness to obtain additional licenses
