Licensed Telemedicine Physician Partner

Job Summary

A company is looking for a Physician Partner to provide telehealth services across multiple states.

Key Responsibilities

• Conduct telehealth visits with patients referred by Community Health Workers (CHWs), including consultations and medication prescriptions
• Collaborate with CHWs to assess patient needs and facilitate access to Community Health Integration services
• Provide clinical oversight and maintain accurate patient records while promoting preventive care and well-being

Required Qualifications

• Active, unrestricted MD or DO license; NP licenses also welcome
• Minimum of 2 years of experience in family medicine, internal medicine, geriatrics, or preventative medicine
• Commitment to health equity and addressing social determinants of health
• Experience in providing clinical support to CHWs or allied health professionals
• Proficient in telemedicine and navigating various digital tools and platforms
