New Jersey Licensed RN Care Manager

Job Summary

A company is looking for an RN Care Manager specializing in Transplant, working 100% remotely.

Key Responsibilities:

• Assess patients' clinical needs and ensure appropriate care levels and lengths of stay
• Coordinate care plans and monitor services provided, advocating for members throughout their healthcare journey
• Serve as a mentor and subject matter expert, participating in quality improvement initiatives and committee meetings

Required Qualifications, Training, and Education:

• High School Diploma/GED required; Bachelor's degree preferred or relevant experience in lieu of degree
• Minimum of two (2) years clinical experience required
• Minimum of two (2) years healthcare payer experience required
• Active Unrestricted NJ RN License or active Compact License required
• CCM certification preferred
