Key Responsibilities
- Provides enrollment support for new PACE enrollees
- Communicates pharmacy benefit information to network pharmacies
- Coordinates facility discharges to ensure medication reconciliation and delivery
Required Qualifications
- High school diploma required
- Must be registered or eligible for registration with the State Board of Pharmacy
- Minimum of 1 year customer service experience, preferably in a healthcare setting
