Job Summary

Key Responsibilities

• Provides enrollment support for new PACE enrollees
• Communicates pharmacy benefit information to network pharmacies
• Coordinates facility discharges to ensure medication reconciliation and delivery

Required Qualifications

• High school diploma required
• Must be registered or eligible for registration with the State Board of Pharmacy
• Minimum of 1 year customer service experience, preferably in a healthcare setting
