New York Licensed Nurse Case Manager

Job Summary

A company is looking for a Registered Nurse / Case Manager (Remote).

Key Responsibilities

• Complete Case Management Assessment and Care Plan
• Collaborate with the care team to identify patients' needs and document interactions
• Triage incoming phone calls and assist prescribers with medication coordination

Required Qualifications

• Successful completion of an accredited nursing program; Bachelor's degree preferred
• Currently licensed as a Registered Nurse in New York
• Current Certified Case Manager (CCM) certification preferred
• Experience in behavioral healthcare is a plus
• Strong computer skills and experience with Electronic Medical Record systems
