Job Summary

A company is looking for a Social Worker Care Manager to service patients in the Queens, NY area.

Key Responsibilities

• Support the mental and physical health care of patients on an assigned caseload
• Screen and assess patients for mental health and substance abuse disorders, facilitating engagement and follow-up care
• Document patient progress and treatment recommendations in EHR and track clinical outcomes

Required Qualifications

• Minimum of MSW required
• Preferred licensure or provisionally licensed
• Experience and knowledge in behavioral health
• Must possess a valid driver's license and have access to an insured automobile
• Proficiency in using Excel and Word for daily assessments and productivity tracking
