PA Licensed Behavioral Health Case Manager

Job Summary

A company is looking for a Case Manager in Behavioral Health (Eating Disorder) to provide clinical support to members with behavioral health challenges.

Key Responsibilities:

• Engage members telephonically to identify barriers to care related to Social Determinants of Health
• Facilitate the development of care plans by identifying members' strengths, needs, and goals
• Act as a liaison within the member's healthcare team, especially in complex behavioral health situations

Qualifications:

• Active PA Registered Nurse (BCBA preferred), LCSW, LSW, or LPC required
• 3-5 years post-licensure experience in an integrated care or healthcare setting
• Strong understanding of Social Determinants of Health and Motivational Interviewing techniques
• Ability to work independently and resolve complex behavioral, medical, and psychosocial issues
• Exceptional assessment and engagement skills with proficiency in Microsoft Office Suite
